An Admissions Counselor will schedule a registration appointment with you where you will discuss which class schedule best suits your availability, and which financial program is just right for you. The counselor will assist you in completing the registration application provide you with a list of material and books you will need when you start your classes.
Complete admissions form. A parent/guardian must sign if the applicant is under 18.
Photo ID / copy of current driver's license AND social security number card.
Enrollment fee of $50.00.
Applicants must be physically able to do massage work.
Applicants must have a valid social security number.

Applicants should be free of any contagious diseases. Any question regarding this should be discussed with the school director at the time the application is made.
Any applicant who is concerned that a previous criminal history may make him or her ineligible for licensure as a massage therapist may submit a Criminal History Evaluation application to the department along with a $25 fee. The department will evaluate the applicant's criminal history report and provide a written determination as to the applicant's current eligibility for licensure.
